THIRD PARTY AGENCY INFORMATION REQUIREMENTS
The following information is required by our Business Office for EACH semester a student receiving agency funding is enrolled and must be received by the day prior to the first day of class:
1. Name of student2. Social Security number or claim number
3. Designated term (i.e., Fall 10, Spring 10, etc)
4. Billing address and phone number for agency
Please include any or all of the following:
1. Tuition and fees2. Required text books only
3. Supplies (i.e., pens, paper, diskettes, etc.) Limit : $_______
4. $30 Application fee (one time only and is non-refundable)
5. Fire suit rental - $95.00 per semester - Fire & Rescue students only
6. Fire helmet rental - $10.00 per semester – Fire & Rescue students only
We will require an additional authorization for optional books, optional supplies, and small tools if sold thru our Bookstore.
